Afaik for the secret ending you can only give him his heart back if he trusts you. At least that's what was in the guide I used when I was trying to get it (failed ultimately due to not being able to read the Lexicon and missing an elven note somewhere).
So far as I know, the only way to outright betray her is with the Demon or Trickster paths. She does get pissed off if you decide to purge yourself of Mythic powers, and will try to stop you from doing so if you accepted her Profane Gift.
