I had a very similar issue yesterday after a new server update. I dug into the transcoder settings and found that the 'Transcoder default throttle buffer' was set to 0 seconds. After setting it to something like 60 or even 120, the issue was gone. Hope this might help!

It's not affecting stuff that goes via Direct Play - it's only when the transcoder has to kick in. What appears to be happening is when the transcoder has completely transcoded the file, instead of Plex sending the last part of the buffer to the client, it sees the transcoder has exited, and jumps to the next file in the playlist.
