The game itself has difficulty zones. And the the map of the difficulties reveals that it's very organized. You have a low difficulty corridor from Sanctuary to Diamond City, and stuff gets more difficult the further east or south you head way from that corridor. The only exceptions are random encounter points, and some rare fixed encounters.

Health regens A LOT slower when eating/drinking/using Stimpacks. You get better loot because more legendary enemies spawn.
