Once you have reached the Mausoleum, you will come across an Ancient Altar with empty spaces. Those empty spaces need to be filled by the Umbral Gems. Now, 3 of them can be found by completing trials, but the 4th one needs to be retrieved ... one should be in the elevator already, you only need one for it and three for the big door, maybe you threw one on instead of "inserting" it. Sounds bugged, should have only needed 1 for the elevator, and probably shouldn't have been able to insert 2.
